Assumption of Risk and Release: I understand that participation may involve risks including physical activity, transportation, facilities, equipment, other participants, and conditions beyond the ministry’s control. I voluntarily accept these risks and, to the fullest extent permitted by law, release and hold harmless On The Edge Ministries, Inc., Edge Community Church, their directors, officers, employees, volunteers, agents, and event partners from claims arising from ordinary negligence connected with participation. This release does not apply to gross negligence, willful misconduct, fraud, or conduct that cannot lawfully be released.

Medical Authorization: If an emergency occurs and I cannot be reached, I authorize ministry representatives to obtain reasonable emergency evaluation, first aid, transportation, and treatment for the participant. I understand I am responsible for resulting medical costs.

Transportation: If selected below, I authorize transportation in ministry, rented, volunteer, or commercial vehicles and understand that normal travel risks apply.

Participant Responsibilities: The participant agrees to follow safety instructions and ministry conduct expectations. The ministry may restrict or end participation when reasonably necessary for safety. Personal property remains the participant’s responsibility.

Media: Photo and video permission is optional and may be declined without affecting participation.

Electronic Signature: By typing my full legal name, I certify that I am the adult participant or the parent/legal guardian of the minor participant, that the information is accurate, and that I have read and agree to this waiver.
